Hidden custom fields after push upgrade

Our customers were using our managed package. Later on, we added few custom fields into the Account object. We then pushed the upgrades to our Client's orgs. But, after the push upgrade, the newly created custom fields were set us hidden.

What is the reason behind this? What can we do to solve this?

Did you also push page layouts in the upgrade which would show the fieldson the detail page ?

We don't have a custom page layout in our package. The user has to add these fields to their layouts.

By hidden, we mean that Field Accessibility of our custom fields were hidden by default.
