Intermittent invalid_grant errors

We have a custom connected app that uses the REST API. In the last couple of days, we've started getting intermittent invalid_grant authentication failures.

It does not happen all the time, maybe 25% of the time or less. I've also tried using cUrl to authenticate on the command-line and I never see any errors, even running the auth command multiple times in succession.

The connected app is already set to Relax IP restrictionsAll users may self-authorize, and Refresh token is valid until revoked.

Any ideas what's going on?

@Mark, we are experiencing this with one of our client integrating with us. Did you figure out why?

For our case, the client confirmed the setup is the same
The connected app is already set to Relax IP restrictions, All users may self-authorize, and Refresh token is valid until revoked.
The weird part is that it works, but after a the oauth2 is called multiple times and the access tokens are used many times, the oauth stops working. throwing the error
{ "error": "invalid_grant", "error_description": "authentication failure" }
Waited for a few hours, the oauth2 started to work again.
