You have to use the Your web site URL for Success and error where you are going to host the HTML pages 

Same like how you are doing in web to Lead

One more issue: when create a form it's asking to lookup for Created by ID - I have put the my user ID 0052A000009M0y2 but it's not finding that...what else should I do? Without this it's not allowing me to go next step..Any help!
