How to shift the customization between sand box and production instance of Salesforce.com

I need to know we are having an enterprise edition of SFDC, we are using a sandbox for r&d , how can we shift the customization done on Sand box to the production instance.

As of now, the customization done in sandbox cannot be moved directly to Prod. I think the only way is if you can upload your work into AppExchange as application and get it back into prod via AppExchange.

Yes, this (package and load to AppExchange) works for many customizations, but not for new custom fields on standard entities, or mods to standard entity page layouts.
