getting 404 error on outbound message

I have a out bound message invoking when there is change in stage, If stage changes and invokes the outbound message I am getting I am getting (404)Not Found error. Any one face same or similar issue ?

So, salesforce.com send the message to URL you configured, and the server at that address returned a HTTP 404 error. check your URL, check that its publicly reachable.
