Pre-chat form has been set up but its not appearing visitors

HI all, 

 

          I have  implemented   Live agent with my developer account and i am using customer portal not using sites .   I have setup live agent  through implementation guide   and developer's guide ,   I  am able  to chat  without   pre -chat form with my visitors. For gertting  customer details   before chat initate. i would like to Display  Pre-chart form   for visitors, i have been setup pre chart form    through developer guide ,  but it's not appering  to my visitors . I went through   some documents . For submitting Pre- chat form to visitors, we have to go through Account dash board settings in salesforce live agent  operate consoler. is it ?  How  would I submit pre-chat form  through my developer account ,?
