Unable to create contacts from Web-to-Case

I have followed and created a trigger (test code covers 100% and deployed to production) but 

 

it seems not working (no contact is created when Web-to-Case generates a new case)

I have been testing but this it not working.

 

Does anybody have the same issue?

 

Our service uses a web form to communicate with customers and 

 

we would like to keep track those customers by adding them into our contact.

I did further testing and found out that

Web Form requires "email" and "name" input field.

I was using customized first name / last name box so it was failed.
