LexiLoader works well, though I've preferred to use dataloader.io lately: it will actually do lookups based on lookup fields, so it's a bit more advanced as well. It's recently gained the ability to use documents on FTP servers as a source and you can schedule the uploads.

 

Salesforce had an online data loading system that I used a few times too but I haven't been able to find it...
