The login itself is just a change in settings for the Sites instance and then exposing the Visualforce/Apex to make it work. Essentially allows the user to work with the customer user license instead of the public guest one - so the cost itself is just associated with the portal (which your Account Executive can get you better details on).

It depends on the license type you are using. You need to contact your salesforce account manager to get a complete information on this.
There are several pricing models, per login, per user, etc.
As mentioned earlier contact your Account Executive for more specific info.
