Yes this can be done pretty easily (and there are a number of examples in Chatter Labs).
The primary object you will need to work with is EntitySubscription. This contains the parentid (record or person) and the subscriberid (the person who is following a record/person).
To know if someone is following a record/user or not, simply query EntitySubscription where parentid = xxx AND subscriberid = theuserid
