Yes, normaly you can construct a report in the reports tab of salesforce and hit "export to excel"
both formats work, CVS and XLS, then when you are in excel, insert a new row above the data, add the table name, now the data area is formated as the connector expects.
to get the ID, add the ID to the report, by tradition it's done in the first column
