How can I create a form on my salesforce site that when it is submitted it is email to the responsible group?
You can use Visualforce to create a simple form for a Force.com Site page that you include in an iframe on your own site. Then, when the form is submitted, you can include a hidden field to indicate that the source of the data is your site and use a Workflow based on new records with the source of site to send an email.
