How to populate fields from one form to another

we are wanting to do a lookup on a field on a form and then copying all of the fields values from that form to the another form.

Can you explain in detail, what do you mean by form? it is VF page or something else?

We added fields to the Remedyforce Incident form and we are doing a lookup on the ICN Circuit field and need to Auto populate the rest of the custom fields on the Incident form. So we have a custom app. called ICN Circuits that contains the data for the fields on the Incident form. So when you select the Circuit you want it would populate all of the other fields on the ICN section on the Incident form.
