I was created an email template and try to create a email alert from work flow but in the email template lookup field my custom template is not visible, how can i select my custom template.

Check the folder in which you have created the template , have you placed in unified public email templates folder?

i was added to the 'Unified public email templates' and also selecting the same in lookup window but my custom template is not visible

Have you marked that template as Available for use while creating it?
