how to include Realted To field in Activity report?

Hi i would like to create a 'Activity with Account' report and i cant seem to find the column 'Name' & 'Related To'.

Check if you have added those fields in your reportType. Once you add in report it will be available in your report.

 'Activity with Account' report is standard report type no? Can we choose what field to display on standard report type?

No you cannot edit standard reportType, you can create custom ReportType with Account as primary object and select "Each "A" record must have at least one related "B" record." option.
