Authentication Failed: OAuth login invalid or expired access token error

I have integrated my heroku apps in sales force using canvus, every thing was fine , apps were wroking fine in salesforce, but now when i tried to acces my apps in salesforce it's giving the following error 

Authentication Failed: OAuth login invalid or expired access token error

 

what can be the possible reason of this issue, Guys help.

In the connected apps oAuth settings make sure that you have you have selected Perform requests on your behalf at any time (refresh_token) in the Selected OAuth Scopes

I'm having the same problem I already have "Perform requests on your behalf at any time (refresh_token, offline_access)" in Selected OAuth Scopes. When first set up the app 2 days ago, it worked fine. Yesterday, it did not. I got "Status 401 - Authentication Failed: OAuth login invalid or expired access token".

I am having the same issue. I did manage to find the OAuth policies. All are set as indicated above and still receiving a failure notice. Any more suggestions? Thanks.
