VF Section Save causes whole case to show

I have this VF section in a standard case page layout.

After I check the box and save, the VF section of the layout shows the whole case page.  Why doesn't it just refresh with showing what was there before save?

it is because you are using the out of box Save of the standard controller,  Create custom method "Custom Save", perform the controller.save and return null;

Yes you do, otherwise you are using standard controller default navigation.
