Create field on report detail

Is it possible to create fields on a report?  In SQL I was able to do this fairly easily.  

An example would be:

Item ----  Qty ---- price --- total (qty*price)

Do I always need to have total on the object or can I create a non-grouped-by report and have a calculated field?

This is not something you can currently do with the reporting engine.  There is an idea [1] on the IdeaExchange to allow for this but it has not been delivered yet but is "coming soon".  Until then you will just have to create a formula field on the object to do this.

are there any tools to improve the reporting in Salesforce?

The only official one would be to purchase Analytics Cloud* [1] from Salesforce.  There are plenty of 3rd party reporting tools available on the AppExchange [2] but I've not used any of them so I cannot speak to them.

* I'm not 100% certian if Analytics Cloud will do what you want here.
