My login is failing, but the user/pass are correct. $uri = 'https://login.salesforce.com/services/oauth2/token'; is this no longer valid?

If you're trying it from Browser:

Try to clear your cookies or try in Incognito mode. I too had this problem then i tried in Incognito mode and it worked fine.

it fails from the command line or any browser.  Stopped working May 19th.
